Introduction
The U.S. Agency for International Development (USAID) Tuberculosis Private Sector (USAID TBPS) Activity supports the Government of Indonesia’s goal of tuberculosis (TB) elimination by 2030 by partnering with public and private sector stakeholders to improve the overall quality of TB care being delivered by private providers. USAID TBPS implements national and district-level, market-based and government-supported Public Private Mixed programming that accelerates private provider diagnosis, notification, and successful treatment to reduce TB incidence and mortality.

Together
with the District Health Offices in activity districts, USAID TBPS will map
current specimen transport systems in each district, including the payment
mechanism. Provider mapping will be linked with diagnostic laboratories, utilizing
Geographic Information Systems (GIS) data to develop user-friendly district-level
dashboards for analysis and improvement of efficient algorithms for specimen
transport.
The
PPM networking strategy will be designed to support the flow of TB care between
private and public sector providers at the district level. Each district’s
networking will be tailored to the specific needs of each district based on the
results of the district situational assessment. The district situational
assessment will be conducted in 9 districts using a phased approach accounting
for each district’s readiness to implement and scale up the project
interventions. The order of data collection will be as follows:

USAID TBPS will focus its data collection among health facilities with a
relatively high number of patients visits, so-called “super-providers”. The
inclusion criteria to be classified as a “super provider” facility includes:
1.
Health facilities that cover two-thirds
of total patient visits in a district (outpatient & inpatient)
2.
Clinics with more than 100 general
patient visits per day (data obtained from District Health Office registries)
3.
GPs (standalone) with more than 25
general patient visits per day
4.
Private health facilities which are
recognized by the DHO or community health centers as facilities with a high
referral of TB patients
5.
All health facilities considered by local
authorities as super-providers for which data is not available from secondary
data collection (Part 1 above)
The district situation activity is comprised of several activities
conducted in sequential order as follows:
1.
Primary Data Collection:
1.
Focus Group Discussions with DHO and Puskesmas to confirm and gain inputs
of the network that is created by the secondary data collection.
2.
Digital survey with health facilities and super-providers. The
digital survey will be performed by enumerators in each activity district to approximately
100-250 health care providers/facilities. Interviews will be conducted with
health facilities’ administrators or supervisors since the questionnaire will
ask about the general profile of health facilities related to TB care performed
by the facilities.
3.
In-depth interviews (IDIs) with DHO, Puskesmas, and selected health facilities
regarding PPM implementation.
2.
Secondary data collection to generate preliminary PPM/sputum transportation
network map collected from government online repositories, health facilities
online repositories, district health offices’ documents and direct phone calls
to health facilities.
TBPS
will hire two Research Assistant consultants to assist with overall data
collection in this activity. The role of the Research Assistant will be to
support the process of primary and secondary data collection throughout the study
in each district.
