Strategic Purchasing Program Lead, Indonesia (Re-advertised)
Palladium
is appointed by USAID to manage USAID-funded Health Technical
Assistance Project (BANTU), to support USAID Indonesia’s Country
Development Cooperation Strategy (CDCS) by
rapidly leveraging specialized short and long-term technical assistance
to USAID, USAID-funded programs; the Government of Indonesia (GOI), and
other organizations and agencies as required.
JOB SUMMARY:
Indonesia’s Jaminan Kesehatan Nasional
(JKN) is now one of the largest national health insurance programs in
the world. The Government of Indonesia aims to achieve universal health
coverage
(UHC) by 2019. However, the country is facing fiscal constraints as
demand for and utilization of health care expand under JKN.
Expenditures on JKN are increasing more rapidly than revenues, and
financial sustainability has emerged as a concern. Improving
the efficiency of JKN expenditures is necessary for making progress
towards UHC, and there is an imperative to make better use of existing
funds through strategic purchasing of JKN services.
For
strategic health purchasing to be effective in Indonesia, a holistic
approach is needed that addresses all financing sources and services,
including priority health services such as tuberculosis
(TB) and maternal and newborn health (MNH) programs. Whether and how
MOH line-item budgets should be integrated into JKN, and which of the
currently donor-supported programs should be funded through JKN, remain
open questions. For all priority programs,
there are many questions about the most effective ways to use provider
payment incentives to encourage providers to focus on prevention,
primary-level services, and efficient, high-quality service delivery.
As
part of USAID’s ongoing health system strengthening work in Indonesia,
the Health Finance and Governance project (HFG) is tasked with
supporting strategic health purchasing within priority programs
in Indonesia’s health sector. With support from HFG and the World Bank,
Results for Development (R4D) is seeking a local Strategic Purchasing
Program Lead to facilitate a process that will result in improved
strategic purchasing for Indonesia’s TB and MNH
programs.
The
Program Lead will provide skilled facilitation of two high-level
technical working groups on strategic health purchasing; expert
technical guidance for research and analysis around TB and MNH
financing; and day-to-day program direction in Indonesia.
RESPONSIBILITIES:
The
Strategic Purchasing Program Lead will be responsible for a variety of
programmatic and organizational tasks that include, but aren’t limited
to:
1.
Provide
expert facilitation of two high-level Strategic Health Purchasing
Technical Working Groups (TWGs), convened respectively by the TB and MNH
program units within
the Ministry of Health (MOH) and consisting of representatives from the
MOH, National Health Insurance Agency (BPJS), and other government
bodies. The two TWGs will discuss issues related
to payment for TB and MNH services; commission
analyses as needed to better understand the consequences for TB and MNH
programs of current purchasing and provider payment arrangements;
oversee
development of options for revised purchasing mechanisms that could
achieve greater efficiency and quality improvements within TB and MNH
programs; and identify key indicators for routine monitoring of the
programs’ performance.
2.
Design
and oversee research and analysis to inform the deliberations of the
TWGS. This could include analyses of funding sources, funding flows,
and payment arrangements
for MNH-related programming; research on patient care-seeking and
referral pathways for MNH care and how these are affected by purchasing
mechanisms; analyses of JKN-related service utilization, claims,
referrals, and total spending for TB and MNH services;
and/or review of billing systems to assess collection and use of
quality of care data. The analytics will aim to identify where provider
payment incentives are misaligned and where improved strategic
purchasing for each program may be possible. The Program
Lead will hire consultants, staff, or subcontractors as needed to
conduct the analyses.
3.
Liaise
effectively with senior government counterparts, maintaining cordial
and responsive relations while advancing program objectives; organize
and attend frequent
meetings at MOH and BPJS to keep the process on track.
4.
Manage
the preparation of technical inputs for review by the TWGs; and manage
the production and dissemination of outputs developed by the TWGs.
5.
Organize
and participate in regular virtual meetings with the R4D technical team
based in the USA and maintain frequent and clear electronic
communications.
6.
Maintain regular communication with Indonesia-based World Bank and USAID funders for the activity.

REQUIREMENT:
Master’s degree or PhD in health economics, public health, public/social policy, public administration or similar,
10 or more years working experience that demonstrates expertise in
health financing and quantitative data analysis, excellent group
facilitation skills and ability to promote effective group
problem-solving and build consensus, strong project and people manager,
highly collaborative engagement style with senior government officials,
strong writer and oral communicator, in-depth knowledge of the
Indonesian health system, key health system actors, and JKN, and
experience in working with external donors and the Ministry
of Health and BPJS-K is highly desired. Fluency in oral and written
English and Bahasa Indonesia is required.
