Nutrition
International (formerly the Micronutrient Initiative) is a global organization
dedicated to delivering proven nutrition interventions to those who need them
most. Since 2011,
Nutrition International (NI) has been supporting the salt iodization program in
Indonesia. This includes development of salt iodization business models, establishment
of quality assurance (QA)/
quality control (QC), monitoring and capacity
building of salt processors in the provinces of Central Java, East Java and West
Nusa Tenggarra (WNT). Indonesia is a large country consisting of a large number
of islands and a long coastal belt where raw salt is produced. It is difficult
to ensure that all local salt is iodized following the national iodization
standard and only production level efforts will not lead to USI in the country.
Although, most of the salt processors registered with SNI in these provinces
reportedly produce adequately iodized salt[1], there is a reasonable
quantity of inadequately iodized salt still being sold in the markets[2] which is primarily
consumed by the poor and vulnerable population. The foremost reason of this is
lack of quality monitoring of iodized salt and insufficient awareness among
community as a result of which some market players take advantage. Hence, there
is a need to involve community groups in monitoring salt that they are
consuming. In absence of market/ consumer level iodization data, it becomes
difficult for province and district level GAKI[3] teams to ensure effective
enforcement so that distributors, wholesalers and retailers sell adequately
iodized salt. To ensure that the community is able to differentiate between
adequately and inadequately iodized salt, NI would like to undertake a
demonstration initiative for the same, which will be done in selected
districts. This demonstration will strengthen community monitoring.
It’s important to note here that
every village receives funds directly from the central government for village
development. The head of village can use these fund to develop and strengthen
the monitoring mechanisms for iodine content in edible salt. At village level
NI is revitalizing the ‘village level GAKI team’ that consists of 5 persons
i.e. head of the village, three health cadres (PKK members) and one village
police which at times monitors the iodization of salt but not regularly.
Therefore, efforts could to be made to engage village level GAKI team to carry
out the monitoring activities i.e. collecting and analyzing salt samples from
village kiosks and retailers by using mini laboratories and report to the
enforcement authorities in case salt samples are found to be substandard.
Considering that women are an integral part of village health cadres (PKK) most
of the GAKI team members are women who work under the overall supervision and
coordination of the village head. They primarily work as volunteers and are
also part of POSYANDU (village level health centers/services), it would be
relatively easy to engage and train them on how to monitor adequately iodized
salt at village level.
The main objective of this
intervention is to support the local government to improve monitoring and
enforcement of USI regulations through establishing women led community
monitoring system in 60 villages in
Semarang City and Tegal District Central Java.
